First-order accumulator for the string-body scan. `Going` carries the decoded
prefix builder and the current remaining bytes; `Done`/`Failed` are absorbing
so the remaining fuel steps are O(1) no-ops (the runtime-graph evaluator runs
every fuel step, so the loop must terminate itself in the accumulator rather
than by short-circuiting the fold).
